Request Form

Fill out the online FOIA Request Form(preferred)

The FOIA Request Form is not required, however your request will be processed optimally if your formal FOIA request contains the following information:
  • Your Name and Organization
  • Multiple forms of contact information (i.e. email, phone, address)
  • A detailed description of the information you are looking for.
In most instances the Village is required to respond to your request within 5 business days. A standard per-page copying fee of 15 cents per page applies after 50 pages. A deposit may be required for large requests. To learn more about FOIA visit the Attorney General's FOIA website.
